The Protocol Intelligence Maturity Model: Five Levels, One Honest Question

Before any AI roadmap, locate yourself. Most clinical organizations  including very sophisticated ones still  sit at Level 1 or 2. The honest placement is the strategy.


AI strategies in clinical development tend to fail the same way: they describe a destination without admitting a starting point. So before capabilities, budgets, or vendors, we ask organizations to place themselves on a five- level curve. It takes ten seconds and it’s clarifying  often uncomfortably so.


The five levels


  • Level 1 — Document. The protocol lives as PDFs and spreadsheets. Every function interprets manually; knowledge lives in people’s heads and meeting notes. This is where most of the industry operates today.

  • Level 2 — Searchable. Text search and chatbots over documents. Faster answers, zero structure: nothing is traceable, nothing persists, and amendments still trigger a full manual re- read. Many teams that believe they’ve "done AI" are here.

  • Level 3 — Structured. The protocol is extracted into a knowledge graph: visits, procedures, endpoints, and eligibility as traceable entities. Questions become queries. This is the level at which AI answers become auditable.

  • Level 4 — Intelligent. AI agents work on top of the graph: drafting deliverables, running amendment impact analysis across every downstream document, surfacing operational insights. Interpretation stops being a human bottleneck.

  • Level 5 — Enterprise. The foundation is reusable across every study, function, and the portfolio. Each new protocol onboards by default; each amendment compounds the asset instead of taxing the team.


The two honest findings


First: most organizations sit at Level 1–2. That is not criticism, it’s the industry baseline, and knowing it beats pretending otherwise. Second, and more useful: the jump from Level 1–2 to Level 3–4 does not require a transformation program. It requires structuring one protocol properly. A single 8- week pilot builds the graph, stands up the AI, and runs amendment analysis on a real study taking one corner of your organization to Level 3–4 while everything else continues as normal.
